Replace a lost or stolen Indian passport from within India
This guide is for an adult applying from within India to replace a previously held ordinary Indian passport that was lost or stolen. Passport Seva treats a lost passport as a re-issue case. Its current document guidance calls for the applicable address and identity documents, an original police report, a lost-passport affidavit (Annexure F), and copies of the old passport pages if available; other documents depend on the applicant's case. The official application instructions list lost/stolen replacement as ineligible for Tatkaal, so use the current portal's Normal application path. Cases involving a passport lost abroad, a minor applicant, damaged but available booklet, diplomatic/official passport, or a special legal situation need the appropriate Passport Office or consular instructions instead.

Report the loss and protect the missing passport details
If the passport was stolen or its loss may involve identity misuse, promptly contact local police and follow the police station's reporting process. Keep the report or acknowledgement and note the date, station and circumstances: Passport Seva's Annexure F asks when and where the passport was lost and when the FIR was lodged. Do not post a scan, passport number, address or police report in a public checklist or support forum. If the passport may still be found, do not delay required reporting or use an unofficial service that promises to cancel or replace it. If you are outside India, a minor, or the passport is damaged and still available, stop here and follow the relevant Passport Office or Indian mission/consulate guidance; this page covers an adult replacing a lost or stolen ordinary passport while applying from India.

Confirm that lost/stolen re-issue is the correct application case
Navigate directly to the official Passport Seva portal at passportindia.gov.in, register or sign in, and select Apply for Fresh Passport/Re-issue of Passport. Because you previously held an ordinary Indian passport, choose the re-issue service and select Lost/Stolen Passport in the document advisor; do not treat it as a first-ever fresh passport. Review the portal's current applicant category and document options for your age, employment, present address, Normal application and other circumstances. Use only the official domain and avoid agents or lookalike sites that solicit passport details. The issuing Passport Office may request additional evidence after reviewing the case, so the dynamic document advisor and jurisdictional Passport Office instructions take precedence over any static checklist.

Build a private case-specific document checklist
Use the Passport Seva re-issue document advisor with the Lost/Stolen Passport case selected. The official application booklet's lost/stolen row lists proof of present address, proof of date of birth, an original police report, a self-attested copy of the first two and last two pages of the old passport if available, and Annexure F, the affidavit describing how and where it was lost or damaged; it also says other documents are required as for the applicant's fresh-passport category. Confirm each item and any extra local requirement in the live advisor rather than relying on this summary alone. Keep originals and copies in a secure folder.

Submit a Normal re-issue application and attend the appointment
Complete the official online re-issue form using the details of the latest lost passport in the existing/lost passport section. Enter the loss information accurately and consistently with the police report and Annexure F; correct errors through Passport Seva's stated process rather than guessing. The current Passport Seva instructions list lost/stolen passport replacement as not eligible for Tatkaal, so use the Normal application option and verify the live portal's case-specific directions before paying. Save the application reference, receipt and appointment details. Attend the booked Passport Seva Kendra or Post Office Passport Seva Kendra with the originals and the required self-attested copies; keep the police report and identity documents protected while travelling. If an official asks for further records, use the secure channel and deadline they provide.

Follow the application and secure the replacement passport
Use the official Passport Seva portal's status service with your application file number and date of birth, and respond to any query through the instructions shown by the Passport Office. Police verification or additional review can apply in lost-passport cases; do not assume the appointment itself means the passport is approved. If the status is unclear, contact the official National Call Centre or the jurisdictional Passport Office listed on Passport Seva, quoting only the requested application reference through that channel. When the new passport arrives, verify its particulars before travel, store it securely, and keep the application receipt and police report under appropriate access controls. Completion means the issuing authority has completed the case and you have checked the replacement passport; this guide cannot promise a processing time or approval outcome.
